Behavioral Health Therapist/Case Manager- OB/GYN
PURPOSE OF THIS POSITION
The purpose of the BVMP Behavioral Health Therapist/Case Manager is to provide a range of psychological assessment, diagnostic, therapeutic, and/or case management services for individuals, couples, groups, and/or families, as appropriate to the specific expectations of the position.
JOB DUTIES/RESPONSIBILITIES
- Duty 1: Provides counseling, therapy, and/or psychotherapy to individuals and families as appropriate to the position; prepares treatment plans, discharge plans, and follow-up care programs; provides therapeutic crisis intervention and emergency services as required.
- Duty 2: Participates in department patient centered meetings, communicates as needed with staff and physicians to enhance continuity of care.
- Duty 3: Participates in various committees and meetings to contribute information regarding utilization of services and quality of healthcare for the purpose of improving patient care and outcomes. Including substance use disorder needs.
- Duty 4: Counsels with patients and families as indicated to present behavioral options toward enhancing life skills
- Duty 5: Guarantees the safety of patients by mandated reporting guidelines for profession.
- Duty 6: Helps to maintain accurate information about provider agencies and community resources to better assist with appropriate linkages or patients and families to these services.
- Duty 7: Will implement process improvement determined to continually enhance quality of services provided as needed.
- Duty 8: Collects information about patients through interview, case history, psychological tests, and/or observational techniques; evaluates information to identify causes of problems and to determine proper therapeutic approach or referral to other specialists.
- Duty 9: Consults with other therapists and related professional and paraprofessional staff, as appropriate, in the performance of therapeutic and/or casework; refers clients to appropriate service agencies as required.
- Duty 10: Participates in the review of clinical issues and program policies and procedures.
- Duty 11: Follows all safety protocols and maintains client confidentiality.
- Duty 12: Performs miscellaneous job-related duties as assigned.
REQUIRED QUALIFICATIONS
- Current license in the State of Ohio as LISW or LPCC
- This position may not be inclusive to the Blanchard Valley Medical Practice (BVMP) that you are assigned at the time of hire, which may require you to work at another BVMP Women’s Health location, as needed.
- A valid driver's license is required (if you do not have a valid Ohio driver’s license you must obtain one within 30 days of your residency in the state). You must also meet BVHS's company fleet policy and insurance company requirements, and any other requirements that may be required to operate a vehicle.
- Positive service-oriented interpersonal and communication skills
- Individual must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served on his/her assigned unit/department. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient status. Must be able to interpret the appropriate information needed to identify each patient’s requirements relative to their age-specific needs and to provide the care needed as described in the area’s policies and procedures
PREFERRED QUALIFICATIONS
- Healthcare experience
- Familiarity with medical office practices
PHYSICAL DEMANDS
This position requires a full range of body motion with intermittent walking, lifting, bending, squatting, kneeling, twisting and standing. The associate will be required to walk for up to one hour a day, sit continuously for six hours a day and stand for one hour a day. The individual must be able to lift twenty to fifty pounds and reach work above the shoulders. The individual must have good eye-hand coordination and fine finger dexterity for simple grasping tasks. The individual must have excellent verbal communication skills to perform daily tasks. The individual must have corrected vision and hearing in the normal range. The individual must be able to operate a motor vehicle for business travel and community involvement.
This position is classified “at risk” for possible occupational exposure to blood borne pathogens (HBV, HIV, etc.)
