MDS Nurse Coordinator Supervisor

Birchaven Village Findlay, OH (Onsite) Full-Time

PURPOSE OF THIS POSITION   

The primary purpose of this position is to coordinate the MDS and care planning process for all assigned residents and oversee the work duties of the MDS Nurses . The Coordinator p articipates in resident and family teaching and provides leadership by working cooperatively in maintaining standards for professional nursing practice in a clinical setting and in accordance with current applicable  standards and regulations.   

JOB DUTIES/RESPONSIBILITIES

Duty 1: Coordinate and oversee the MDS care planning processes.  Participate in resident and family education and support.   

Duty 2: Supervise the MDS nursing staff and provide direction and education to them.   

Duty 3: Conduct and/or direct the interviewing of each resident for the resident’s assessment and ensure that an initial assessment is completed within 14 days of admission and care plan is completed within 21 days of admission.  

Duty 4 : Work with the interdisciplinary care plan team in developing a comprehensive resident assessment and care plan for each resident, including the assessment of nursing needs and utilization of forms required by current regulations and facility policies.  

Duty 5 : Coordinate the development of a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care.  

Duty 6 : Coordinate the quarterly review and revision of care plans by the interdisciplinary team to assure that the care plan is evaluated and revised after each assessment or change in status.  

Duty 7 : Establishes and maintains communication to all other departments involved in the MDS process and utilizes teaching opportunities to provide for the specific learning needs of each resident and their significant other(s) during care conferences.  

Duty 8 : Demonstrates the knowledge and skills necessary to complete MDS assessments and plan of care, ensuring that each portion of the assessment is signed and dated by person completing, and understands Medicare and Medicaid rules and regulations.  

Duty 9 : Track and submit MDS data according to State regulations.  

Duty 10 : Monitor case-mix and scoring to ensure accuracy of assessment.  

Duty 11 : Monitor quality measures on monthly basis . Complies with organizational policies, procedures, and practices to ensure quality job performance and regulatory compliance.  

Duty 12 : Displays service excellence and the standards of behavior as evidenced by practicing the mission, vision, and values of the organization to promote resident satisfaction.  

REQUIRED QUALIFICATIONS

  • Current Ohio RN license  
  • Maximum exposure in regard to universal precautions  
  • Positive service-oriented interpersonal and communication skills required.  
  • Individual must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served on his/her assigned unit/department. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient status .  Must be able to interpret the appropriate information needed to identify each patient’s requirements relative to their age-specific needs and to provide the care needed as described in the area’s policies and procedures  

PREFERED QUALIFICATIONS

  • 1-2 years of experience in long-term care is preferred.  
  • 1-2 years of experience coordinating assessment dates and transmission of data.  

PHYSICAL DEMANDS  

This position requires a full range of body motions with intermittent walking, lifting, bending, squatting, kneeling, twisting, and standing. The associate will be required to walk for up to one hour a day, sit for four to six hours a day and stand for one hour a day. The individual must be able to lift twenty to fifty pounds and reach work above the shoulders. The individual must have good eye-hand coordination and fine finger dexterity for simple grasping tasks. The individual must have excellent verbal communication skills to perform daily tasks. The associate must have corrected vision and hearing in the normal range. The individual must be able to operate a motor vehicle for business travel and community involvement.  

This position is classified “at risk” for possible occupational exposure to blood bor ne pathogens (HBV, HIV, etc.)
